If the QA is insisting ".5" after weight instead of "1/2", wouldn't they also insist on typing "A 2.5-year-old child" instead of the obviously correct "A 2-1/2-year-old child?"  It just doesn't jive with me on some level, but then again neither did Aqua-Net hairspray.      

Clear as a bell, but I have no clue!  no intrinsic craniovascular bruits.

I have tried every possible spelling of this word, med. dictionary, google, webmd. I can't seem to come up with it.

The context doesn't give much away either.  One is the problem list, one is the past medical history, and one is:

There is a history of ... and he had a PEG placement.

Not even sure if the two go together, LOL, she tends to run her sentences together.

I was taught to never change a transcription, even if it's blatantly wrong.  I would leave as dictated and leave a note for the doctor (if you have a place to do it)

I hate it when the doctors/PAs prounounce diabetes MELLitus as diabetes mellITUS like it's some kind of inflammation or something.

Good grief - you're a highly educated person.  You should know how to pronounce the medical terminology and medications.  Sorry.  Just tired and crabby todayd. 

. I think it might be quietapine (Seroquel) and the dose is titrated etc. due to patient's other problems etc.  All I can think of.

result can greatly affect the mean, but the median is not as affected.    Any clues?  This is a horrible dictation. 

First typed it back in 1970s. Then again in the 90s and the doctor used all abbreviations. Almost lost it that time!!!  They all have their little "docisms". keratinaceous is used by a lot of them. For real!!!

s/l:  "Also, given the patient’s financial status, I believe this is of less yield."

Does this make sense to anyone?  I have listened to it a hundred times and it's making me bonkers!

Doc says, Her s/l NVOS was running between 53% and 56% which correlated with her mixed venous saturations. 

I had blanked this morning and QA has blanked it as well, and now I have it again this evening.  Would 'prec whatever help.

At that time, it was decided to explore (s/l trileptory) concerning for resection of small bowel. The patient went to surgery on that day for exploratory laparatomy, small bowel resection of primary anastomosis, and was discharged on 03/18/2009.

(they are concerned about a small-bowel obstruction.)
