The doctor's really could care less what is written BOS.  They are interested in quality work.  It is funny how some companies and transcriptionists breath/live it.  I am sure there are some doctors out there who are counting every key stroke but most just want to practice medicine.  btw....still spacing twice after periods also.....so much for the BOS.....it just doesn't have a whole lot of power these days.

Google.  I have seen more than a few MTs that tend to check s/l words with this search engine and some of the things that get linked together on there can be either frightening or funny at times, depending how you look at it!  Just an over-cautious old MT?

Do I replace L5-S1 with lumbosacral joint? 


My account gives examples that we should expand "Fracture, L2" out to "Fracture, second lumbar" ....  so that is why I'm thinking I need to expand the L5-S1...

Because abbreviations can mean so many things, I never expand abbreviations even in a diagnosis. Our hospital determined it is the dictators job to edit before signing if they want it expanded.

When the doctor dictates "as stated in the HPI" , should I expand that to History of Present Illness - with those caps in place or only capitalizing the H?

Client profile dictates whether or not to give this dictator mets or metastasis.....his saying *met* was wrong.  I would've given him *mets* unless the client profile allows expanding of words, and in that case I would've given them *metastasis*

Okay, I had to gripe.  This is all I get lately.  I have actually broken my own record for finding someone who stutters more than someone I used to work for.  That's a terrible comparison, as everyone loved thiss doc and doc's dictation.

I'm thinking they could be abbreviating a skin condition as "SK," but that could stand for a number of things (seborrheic keratosis [or keratoses], for one). Can't say....

I don't know who they got working there sometimes.  So much conflicting stuff it's unreal.  I call them on it every time though and send it to my super.   It's hard enough without the conflicting feedback.


Example:


Dictator said HPI (for a heading), so I typed it HISTORY OF PRESENT ILLNESS.  I get QA feedback that says, "If the dictator dictates the header as HPI, type it in as HPI."  So I'm like okey dokey.Next time I get HPI I do just that.  Then I get a nasty note.  NEVER, EVER abbreviate in the headers, always spell it out.  I'm  like geeesh!


One told me to type out 2:00 O'clock (for position, i.e., cornea).
