but then I'd also have expected him to be talking about candidiasis instead of strep because he mentioned white substance on lips and oral mucosa.

or even itchy legs. Unfortunately it is possible he is quoting the patient's description. If you are sure, I'd do it as you have it, with the quotes. If you aren't sure, then I'd blank it.
