How would I....sm

This is the sentence that was dictated:

"I outlined surgical repair with reduction of scrotal hernia sac with repair with mesh to repair this successfully and its associated risks and benefits."

It seems hard to read to me, but I am confused on how to make it easier to read or if I should just leave it as it is.
