Does anyone know if "restless legs syndrome" should be hyphenated like restless-legs syndrome?  Our department is really cracking down on QA so I need to know for sure.

I'm doing an H&P on an obese patient with facial suffusion and a 2-week history of fatigue, 15-pound weight gain, peripheral edema, dyspnea, dizziness, and tingling.
In the assessment, he says historical and physical exam findings consistent with s/l tic leukian or tic loo can syndrome. I can't quite figure this out, and my spelling must be too far off to catch a clue. Can anyone point me in the right direction?

I work for vascular surgeons and I've never heard of any veins in the legs that even sound like jugular veins.
I would expect him to say restless legs

or even itchy legs. Unfortunately it is possible he is quoting the patient's description. If you are sure, I'd do it as you have it, with the quotes. If you aren't sure, then I'd blank it.

I would type it under PLAN and type what is dictated.

I have had that happen to me before, and I was told by QA to not move things around in the report, maybe that is the same for you, but if I were typing this report, I would type it in the order as is dictated, even if that means typing it under the heading PLAN.  Also, his vitals were probably taken at the end of the exam as they were repeated, so it would make sense to type them again under that heading.  Hope that helps!

doc doing consult says what s/l seca, sica, sacka syndrome (dry mouth and dry eyes). Doesn't sound at all like Sjogren's which is all I can find for dry mouth and dry eyes syndrome.
