A "5-pack-year" history does not mean "5 packs per year" ... good grief, I'd consider someone who smoked 5 packs per year essentially a nonsmoker. :-)
A 5-pack-year history means an average of a pack a day over a 5-year period. That could mean someone smoked 5 packs per day for 1 year, or 1 pack per day for 5 years, or half a pack a day for 10 years, or any other combination that fits.

sell another version of the BOS. I go with client preferance, which is still spelling out 1-10 and numerical above 10. Dates are spelled out within the body of the report...client preference. AAMT is not the know all end all, just another money grabbing gimic! JMO
