I don't have a clue what this doctor is saying.  The patient has a history of ____ malformation with subsequent surgery.  Since surgery, she has had a tongue that deviates to the left.  It was crooked due to muscle atropy and ____.


The blanks sound like "guillare malformation."  It actually sounds like the doctor is saying G-R-E malformation.  At one point the G has a hard sound, but when he says it again he uses a soft G sound (like J).

I can't find anything on it but it sure sounds like the doctor is saying radioulnar collateral ligament instead of radial and/or ulnar collateral ligament. The patient has had a long finger trigger digit injection and has pain on the volar aspect of the middle phalanx.

Past history says: She had four cesarean sections, laparoscopic tubal sterilization, and a laparotomy with left oophorectomy and abdominoplasty.

In pelvic exam: PELVIC: Skene, urethral, and Bartholin glands within normal limits. Vagina, good support. Cervix was closed. No lesions were seen. Uterus was palpable, ______. s/l "ring incised" or "ring sized."

Could he be saying "ring incised" meaning he feels the incision from previous surgeries. Could it be he is saying uterus is "ring sized?"
