Vicodin 5 per 500 or 5/500
I was wondering if anyone knew if the dose for a medication such as Vicodin would be written as 5/500 or would it be 5 per 500 (as BOS). I have been writing it as 5 per 500 but the doctor says in his mind that does not make much sense. Am I misunderstanding what BOS is saying. Thanks in advance.

this is just in the med list, no real history on the patient. I've tried all of my resources, but I cannot seem to come up with anything. Sounds like the same way a doc would dictate Combivent, but it is not that. I wonder if someone has a link for looking up drugs by the dosage??? long shot, but trying here... Thanks in advance.
