suggestions on how to transcribe 0 negative 3 positive
Subject: suggestions on how to transcribe 0 negative 3 positive

This is dictated in a neuro operative report on a patient with Parkinson disease.  The doctor is doing insertion of bilateral deep brain stimulators:

Macro stimulation using 0 negative 3 positive pulse width of 90, frequency 145 resulted in tingling of the left side at threshold of 2.5.

or would it be easier to understand like this?

Macro stimulation using 0-/3+ pulse width of 90, frequency 145 resulted in tingling on the left side at threshold of 2.5.

I honestly don't really understand what I'm transcribing here, so maybe a comma should be after the 0-/3+.

Subject: gram-negative, lactose-negative rods is correct. I found a couple of interesting websites SM

after doing a google search for gram-negative, lactose-negative rods.  Basically testing organisms to see if they ferment lactose (positive) or not (negative).

Also, just FYI, gram-negative is not capitalized.

as no HUMAN name is before it...sm
Subject: as no HUMAN name is before it...sm

the original poster said something like "pt to be seen by cardiology and infectious disease departments" -

These are not proper nouns/names..........it would only be a proper noun/name if the hospital name precedes it.....i.e., St. Mary's Dept of Cardiology and/or St. Mary's Infectious Disease Department, or a name like....John Doe's Dept of Cardiology etc., etc.

Unless, of course, your client wants these things capitalized...but even if your client wants them capitalized, without a proper name preceding the departments, your client would be incorrect, but again one gives the client(s) what they want.

Subject: basic human anatomy (thumb has "opponens" musculature) sm

Opposition is the movement whereby the palmar aspect of the thumb touches the palmar aspect of another finger of the same hand. (When the contact is not by their palmar aspects, the movement is merely apposition.) The converse of opposition is reposition. These movements occur at all the joints of the thumb, although opposition takes place mainly at the carpometacarpal joint.(source--Dartmouth.edu)
