Subject: HPI

I  have an HPI report.  I've indicated all the headings.  The last one is PLAN.  At the very end the doctor dictates this:  His repeat vital signs:  His heart rate was down to 197.  His blood pressure was 124/86, and he was able to stand and move with greater ease.  (Does that fall under the plan or do I need to seperate itunder another heading)  What would that be?  I've searched but cannot figure it out.  I'm a newbie so I am constantly questioning everything.
