I was corrected when I typed in Grave disease but according to BOS rules for posessive forms you should omit the 's unless the noun is omited following the eponym, eg. Grave disease or Grave's or Alzheimer disease or Alzheimer's.  Is Grave's an exception to the rule?  Is there a steadfast rule that I should follow on this as to what follows the rule and what doesn't and if there is, how do I remember?

In a cataract extraction, would a haptic or an optic be captured in the ciliary sulcus? He later says that the optic was captured in the capsular bag with the haptics being latched securely in the sulcus.

I don't exactly know what you are asking.  If the question is regarding what he meant and you have any question, flag it and send it to your Q.A. or whereever you send reports with blanks.  Not a good idea to try to "correct" the doc's dictation and an even worse idea to guess.

Once again need some ideas. I swear it sounds like she is saying hemnio-optic but no work exists like that...

The context is: "...her history of Alzheimer’s and her visual field defects which are ??optic in nature..."

Any ideas?

Lab tests done at your office on 08/01/07 indicated a free T4 of 4.36 (0.8 to 1.9), T3 was 390 (72 to 170), and a TSH of less than 0.01.  A WBC count was slightly low at 3.5 (3.9 to 11.3), hemoglobin was 12.5, hematocrit 37.6, _____ of 77 (80 to 102). 


The blank sounds like LEV-IN AND CBU of 77.


??? any thoughts on what lab value this could be based on the normal range of 80 to 102?  thanks.

I have searched and searched and can't figure out what he is trying to say. 


She does not have any of the typical findings on exam; she may have a s/l peer small-fiber neuropathy.  I will see her again after EMG studies.  The work-up of s/l peer sensory neuropathies is arduous
