Use


Treatment of bronchospasm, anaphylactic reactions, cardiac arrest, and management of open-angle (chronic simple) glaucoma


Usual Dosage



  Bronchodilator:

  Children: S.C.: 10 mcg/kg (0.01 mL/kg of 1:1000) (single doses not to exceed 0.5 mg); injection suspension (1:200): 0.005 mL/kg/dose (0.025 mg/kg/dose) to a maximum of 0.15 mL (0.75 mg for single dose) every 8-12 hours

  Adults:

  I.M., S.C. (1:1000): 0.1-0.5 mg every 10-15 minutes to 4 hours

  Suspension (1:200) S.C.: 0.1-0.3 mL (0.5-1.5 mg)

  I.V.: 0.1-0.25 mg (single dose maximum: 1 mg)

  Cardiac arrest:

  Neonates: I.V. or intratracheal: 0.01-0.03 mg/kg (0.1-0.3 mL/kg of 1:10,000 solution) every 3-5 minutes as needed; dilute intratracheal doses in 1-2 mL of normal saline

  Infants and Children: Asystole or pulseless arrest:

  I.V., intraosseous: First dose: 0.01 mg/kg (0.1 mL/kg of a 1:10,000 solution); subsequent doses: 0.1 mg/kg (0.1 mL/kg of a 1:1000 solution); doses as high as 0.2 mg/kg may be effective; repeat every 3-5 minutes

  Intratracheal: 0.1 mg/kg (0.1 mL/kg of a 1:1000 solution); doses as high as 0.2 mg/kg may be effective

  Adults: Asystole:

  I.V.: 1 mg every 3-5 minutes; if this approach fails, alternative regimens include: Intermediate: 2-5 mg every 3-5 minutes; Escalating: 1 mg, 3 mg, 5 mg at 3-minute intervals; High: 0.1 mg/kg every 3-5 minutes

  Intratracheal: Although optimal dose is unknown, doses of 2-2.5 times the I.V. dose may be needed

  Bradycardia: Children:

  I.V.: 0.01 mg/kg (0.1 mL/kg of 1:10,000 solution) every 3-5 minutes as needed (maximum: 1 mg/10 mL)

  Intratracheal: 0.1 mg/kg (0.1 mL/kg of 1:1000 solution every 3-5 minutes); doses as high as 0.2 mg/kg may be effective

  Refractory hypotension (refractory to dopamine/dobutamine): I.V. infusion administration requires the use of an infusion pump:

  Children: Infusion rate 0.1-4 mcg/kg/minute

  Adults: I.V. infusion: 1 mg in 250 mL NS/D5W at 0.1-1 mcg/kg/minute; titrate to desired effect

  Hypersensitivity reaction:

  Children: S.C.: 0.01 mg/kg every 15 minutes for 2 doses then every 4 hours as needed (single doses not to exceed 0.5 mg)

  Adults: I.M., S.C.: 0.2-0.5 mg every 20 minutes to 4 hours (single dose maximum: 1 mg)

  Nebulization:

  Children <2 years: 0.25 mL of 1:1000 diluted in 3 mL NS with treatments ordered individually

  Children >2 years and Adolescents: 0.5 mL of 1:1000 concentration diluted in 3 mL NS

  Children >2 years and Adults (racemic epinephrine):

  <10 kg: 2 mL of 1:8 dilution over 15 minutes every 1-4 hours

  10-15 kg: 2 mL of 1:6 dilution over 15 minutes every 1-4 hours

  15-20 kg: 2 mL of 1:4 dilution over 15 minutes every 1-4 hours

  >20 kg: 2 mL of 1:3 dilution over 15 minutes every 1-4 hours

  Adults: Instill 8-15 drops into nebulizer reservoirs; administer 1-3 inhalations 4-6 times/day

  Ophthalmic: Instill 1-2 drops in eye(s) once or twice daily

  Intranasal: Children ≥6 years and Adults: Apply locally as drops or spray or with sterile swab

  Bitartrate (AsthmaHalerŽ, BronitinŽ): 0.3 mg/spray [epinephrine base 0.16 mg/spray] (10 mL, 15 mL, 22.5 mL)

  BronkaidŽ: 0.5% (10 mL, 15 mL, 22.5 mL)

  PrimateneŽ Mist: 0.2 mg/spray (15 mL, 22.5 mL)

  Auto-injector:

  EpiPenŽ: 1 mg/mL [1:1000] (2 mL)

  EpiPenŽ Jr.: 0.5 mg/mL [1:2000] (2 mL)

  Solution:

  Inhalation:

  AdrenalinŽ: 1% [10 mg/mL, 1:100] (7.5 mL)

  Injection:

  AdrenalinŽ: 0.01 mg/mL [1:100,000] (5 mL); 0.1 mg/mL [1:10,000] (3 mL, 10 mL); 1 mg/mL [1:1000] (1 mL, 2 mL, 30 mL)

  Suspension (Sus-PhrineŽ): 5 mg/mL [1:200] (0.3 mL, 5 mL)

  Nasal (AdrenalinŽ): 0.1% [1 mg/mL, 1:1000] (30 mL)

  Ophthalmic, as hydrochloride (EpifrinŽ, GlauconŽ): 0.1% (1 mL, 30 mL); 0.5% (15 mL); 1% (1 mL, 10 mL, 15 mL); 2% (10 mL, 15 mL)

  Topical (AdrenalinŽ): 0.1% [1 mg/mL, 1:1000] (10 mL, 30 mL)
