I've only ever used SNF. Perhaps SNIF is right, but I've never ever heard it, let alone used it. To clarify, I've heard them say sniff, but I've never heard them say the whole phrase spelled out.

The MOST obvious is apparently like I understood it:

Maybe the doctor really meant this, 5 packs per year, so essentially a nonsmoker.

She is thinking of filing a complaint with elder's abuse.


The pt is not happy with her assisted living facility. Any clues on how to transcribe this one. I could not find an actual Elder's Abuse anything.

In med list, doc states Carbetrol 900 mg q. h.s.


There is no mention of seizures, but there is mention of back pain and I noticed that Carbetrol can come in 300 mg tablets, and that it can also be used for trigeminal neuralgia, which there is no mention of either. This is an ESL, so it is posisble that I am not hearing right, but it was never mentioned again. Past medical history is bipolar, dyslipidemia, hypertension, and gastritis. Thank you!

I type verbatim, so if the doc dictates of I put it that way, if not, I don't, but have seen QA do this to me many times. To me this is so insignificant and it does not critically change the patient's care in any way, I just let it go. Some of my QA people change it, some don't, wish they'd be consistent, but I really don't worry about it.

Epsilons are not related to the heart size per se.  They're waves that can be seen on an EKG.
