All I know so far is that the patient is being seen in GI consultation and she is combative and age 90+.
REASON FOR CONSULTATION sounds like:

I'm thinking

but still clueless. He is ESL, probably Indian.

It probably is nothing more than a simple "consultation", as sometimes those problems could be managed by Orthopedics alone without the need for surgical intervention...or the diagnoses confirmed and referred to an Orthopedic Surgeon for further care.

I would be hesitant to number them is because of the coding issue. If I'm not mistaken, each diagnosis is coded for insurance rasons, proper billing, etc.

usually he says laboratory data and he did not on this report. He talked about the ECG then just said INR 3.0. The doctor says type as dictated, so I did not feel it went under ECG thats why I wanted to make sure about the lab, not unless it would appropriately fit under ECG?? Thanks
