I'v been doing this for quite a few years, and last night had a report where I heard "Shoreman kyphosis," researched and found quite a few websites spelled this way, didn't really check to see source of websites, just that there were a LOT of them. (slap me with a wet noodle, I was tired and brain-dead because I know better)
Anyway, QA send me an email this morning with correct spelling, "Scheuermann kyphosis." citing Dorlands online.
You can bet I created a shorthand entry for that spelling immediately, and of course, sent of an email to my QA person letting her know how much I appreciated the feedback -- I never have questioned my research without the feedback!

I'm not here to argue the point with you of whether verbatim accounts are a good idea or not, or whether the insurance company, lawyers, or the man in the moon will like it.
