Doc is going over the lungs on an Autopsy Report.
"....The alveolar walls are thickened. The vessels are congested. Moderate numbers of neutrophils are in the ALVEOLAR walls." The problem is, that he sounds like he is saying "NOVEOLAR". But that just doesn't make sense. Any ideas if he meant Alveolar and dictated incorrectly??

What is the correct way to type height measurements. For example, doctor says "5 foot 1 inch". Normally when the number is pleural, it should be "feet", but the doctors always say "foot". Also, is there a comma between the foot and inches. Such as, "5 foot, 1 inch"? TIA

When doing the PE, the doctors usually dictate the fundal height along with the vital signs. I was always taught in the hospitals where I worked that fundal height went along with the abdominal exam. Thoughts?

I found some articles on the internet that have the word pigmentated (even pigmentated Bowen carcinoma) when I googled, but also found pigmented... Pigmentated is what was dictated, so I'm confused. I too could only find pigmented in my books too and not sure if this is misspelled on the internet. Some of those sources are unreliable. Thanks for helping.
