I would separate this from the social history with a new heading unless you're typing an account that is absolute verbatim and no embellishment is allowed.
I try to think of the record from the patient's standpoint - the easier the information is to locate quickly, the faster and more efficient their treatment will be in a pinch. I know I'd want my record treated that way!

Problems: It wraps to the second line and I don't know how to format/word that sentence into a cc line.

is it best to list and number? commas? periods? maybe a colon after given? Just wondering what others might have to say. Thank you. :)

Thank you for the replies. The reason I ask is because it is different with every assignment; depending on who grades my paper. It ticks me off and confuses me.

Looking for any help on formatting transcription labels in Word to print to a laser printer. We purchased labels (3 per page)...I have tried to change settings in Create New Label; however, if we need it to continue on the next label it doesn't seem to work, the text continues, but "underneath" the next label. How can we set it up so that it continues on the next label? Any ideas would be greatly appreciated!

Can anyone help me do the following:
I want to do a numbered list (i.e. an IMPRESSION: section), so that:
The numbers are flush left again the margin; and -
Each entry is indented a few spaces (a few spaces to the right of the numbers in the list). If one entry in the list goes for more than a single line, I want the second line of that entry to ALSO be indented -- so that it does not sit underneath a number and does not look good. Every time I try to use the MS Word numbering feature, it pushes the whole list (including the numbers) to the right, so that the numbers themselves are no longer flush agains the left margin.
Thank you very much. Willing to call someone too if they can help me. Very important.

How would I transcribe this? Would I type, "Patient was prescribed Pyridium?"
