Enclosure question

This is a letter where the dictator says at the end:


"Please enclose last 3 chart notes with letter."


I rarely do letters and am wondering how I note that at the bottom....???


Would I put just "Enc" or do I type "last 3 chart notes"...???

Do you really think the office staff is going to know what Enclosure: 3 means?  If the client on the other end does not know what the doc dictated, then how will they know he/she wants the 3 last chart notes if she just types Enclosure:  3?  JMO.

The doc wrote a letter to a physical therapist on behalf of a patient. At the end, he says to copy to the patient and include a copy of her MRI scan results. How do I transcribe this so that the doctor's office knows to enclose the MRI results to the patient, not the physical therapist who the letter is to?
