i think either one is okay (+0 or plus zero) but using "plus zero" is preferred if given at the beginning of a sentence
but...femoral neck and femoral shaft are 2 different

Pt. had chest pain while walking...then the Dr. dictates " he felt like he was hit in the chest. There was a 'somewhat phoretic component to that'....."  any ideas?  I've tried to make "diaphoretic" out of it, but I do not hear that. Please help and thanks!!!!

The nature of her chest wall spasms is not clear. The first thing that comes to mind is herpes, but this would have to be herpes syn rash. There does not appear to be a cutaneous component to it, and postherpetic neuralgia is less likely but still remains the most likely diagnosis.

The above knee popliteal artery was identified distal in the adductor canal and dissected out behind the knee and encircled proximal and distal with silastic tapes. A large calcification of both vessels was noted. A s/l Skanlin tunneler was used to create a subcutaneous tunnel.
