Dictator states, *The patient notes loose bowel movements especially after a bowel movement.* This guy is kind of absent minded anyway and I'm tempted to flag it but then I'm wondering if there's something I'm missing where this could be what he really means? Just thought I'd ask for opinions since it's at the beginning of a very long report.

Any operation on her would be a fairly significant undertaking being a third hernia repair, both previous ones with mesh. I would attempt this laparoscopically, but I realize there is a high chance that she has intraperitoneal adhesions or S/L: BOWEL inherent to her mesh.
Is that right?

Her family history significant for syncope and ADHD, BUT, does the above sentence itself make sense?

However, it really sounds like she is dictating "Carter pillow" but I had never heard of one before so I really wanted to double check. That most likely has to be it but I'm still debating putting a blank. I'll listen a few more times and most likely use that. Thanks so much! :)
