The skin around the temple area shows small light brown hyperkeratotic lesions which appear to be seborrheic keratosis.  Similar lesions noted over the entire back with (s/l debriding colors)

Drugs that are used to lower blood glucose levels are typically called "oral hypoglycemic agents." If patient has high blood sugar, then you want to bring the levels down with hypoglycemic agents.

I got an ESL doc that I cannot differentiate between him saying hypo and hyper. 


He states that the TSH is more than the normal level and then prescribes levothyroxine.  Isn't levothyroxine for hypothyroidism?

Hypo, hyper, macro, micro
I'm a long-time MT and I still have a problem telling hypo from hyper, hypothyroid from  hyperthyroid,  macro from micro, etc....unless it's very obvious from the content of the dictation which is a lot of times isn't.    Another difficulty is the lab values...it's hard to tell 15 or 50, 20 from 30.  I leave blanks and then I see where QA fills them in.  So, I don't know if my hearing is getting bad or if they or just guessing.  Or, they may have access to othe lab values from previous dictation.  So I'm just wondering if this is a common problem with other MT's. Maybe I need to have my hearing checked.

SKIN:  Lots of seborrheic keratoses.  He does have an elevated 9 mm papule on left side of the face, which has some vascularization and central core.  It almost looks like acanthokeratosis, but basal cell carcinoma cannot be fully ruled out.  No other suspicious lesions.  He does have some miliary rubra on the back.

Assessment:  Nevus s/l: roo-let-ee s/l: tip-e-er midabdomen.  (Nevus ruletti tipiar midabdomen)?? AND:


s/l: Dermatified (or dromatified) broma right shin.  She uses this phrase in a previous sentence:


On his right shin he has superficial change that appears like a dermatified broma; however, it is not as nodular beneath the shin, but it is definitely excoriated or irritated on the surface. 


I just cannot find any info on these two - sorry this is so long.  I am covering for someone and would prefer not to send to the editor!!  TIA
