If it is business correspondence, I would use the person's "official" first name. If it is a personal letter, I would use the nickname.
If you're not sure, I would advise leaving it blank.

While account specifics vary from hospital to hospital, I don't think I've had a hospital with an H&P that didn't require that you separate out the headings; discharge summaries, on the other hand, are usually allowed to be in paragraph form, and consults are a toss-up. Clinic/office notes are totally a matter of account specifics. When you get into specialty clinics, they sometimes break it down even further; i.e., an ENT doctor may separate out HEAD: EYES: EARS:, etc., and a neurologist may do MENTAL STATUS: CRANIAL NERVES: STRENGTH: REFLEXES, etc. However, in most cases, if the doctor uses headings in an exam for some of the systems/body parts, you usually are to use the appropriate ones for all; as noted, though, that is dependent on account specifics.

Subject: Depends on what ports you have on laptop and type of cable you have on the screen...sm
If you have the matching ports and cables, I believe you can do this. I use my regular sized kb on my daughter's laptop all the time. Should be the same concept? If your screen has a usb cable, plug it into the laptop usb port. The laptop should recognize the new hardware and "install" it.
If you have a parallel port, the kind with the prongs that insert into the tower and then screws to tighten, you may have to install a parallel port into your laptop. Don't know if this is as easy to do as on a desktop or not.

Regarding letter h (hour) per BOS...please sm
Letter h. (with a period) is the abbreviation for the Latin word hora, meaning hour.
q.6 h. (every 6 hours)
In instructions for medications, Latin abbreviations may be coupled with English terms, such as hour. These may be transcribed as dictated or the English translated to Latin.
every 4 hours or q.4 hours or q.4 h or q.4 h Copyright (c) 2002 American Association for Medical Transcription. So does this mean we can do the q.4 h without the period at the end of h, because I am constantly having to remove capital letters from words following the h with a period. Thanks.

I am thinking....
In:
2004 225 pounds
2005 242 pounds
etc? However, it would not be double spaced for some reason it keeps doing it on here?

My MD dictated a letter of recommendation and would like me to make him sound better. Gosh!! I am so brain dead when it comes to these things... been doing verbatim transcription for far toooo long.

I appreciate any suggstions. Thanks.
Mr. X spent a week with me in my internal medicine office this past summer. He is currently a student at XX University and has decided on a career in a medicine. Mr. X, in the process of starting his medical career, has spent time with multiple physicians to show that he is serious about a career in medicine. Mr X has excellent personal skills, as you will see when you interview him. He asked intelligent questions and showed a genuine interest while he was here with me in the office. His thought process seemed both logical and rational during our discussions.
Mr. X has all the makings of a good physician, intelligence, hardworking, motivated, and personal. I recommend highly that he be accepted in your training to become an xxx physician.
Need help with letter I am typing re platelet counts
Subject: Need help with letter I am typing re platelet counts
The studies demonstrated carotid artery disease but, also, she has a platelet count of 1004 k per cmm and persistent reduction in her hemoglobin and hematocrit at 9.4 and 31.5.
Does anyone know how to format this? I can't find the K at all in relation to these values, but doc is clearly saying, 1004 K per cmm. thank you.
re: Need help with letter I am typing re platelet counts
Subject: re: Need help with letter I am typing re platelet counts
K is abbreviation for thousands; also used as abbreviation for potassium. platelets are expressed in thousands.
I think the 1004 k/cmm should be expressed this way.

Subject: for a straight consult rep and not real letter
but it DOES depend on the client profile/preference like the other poster said.....I get this format all the time in REAL consult reports (not letters) and I just type *Dear Dr. Smith* - I have just seen your patient....blah blah (can usually go under reason for consult or HPI) and then do my headings as dictated.
Good question, though!

I have an ortho doc that insists on using it the way the dictionary shows it, follow-up. He is not of old school, very young and a professor. Says it is his biggest pet peeve to see it ever used without the hyphen, and doesn't care much for BOS rules.
So, when in doubt, I do it exactly the way the dictator wants it no matter what I think.

It honestly depends on your employer's guidelines. Check with your Editor/QA staff. Sometimes there is just no way to make a doc make sense on paper without changing everything around and so it's easier just to really type verbatim; some docs only need a tweak here and there, so you really need to find out what 'verbatim' means as applied to the account you are working on.
