Is the hypen in mm and cm placed when the structure or fluid being measured is mentioned after the size?  For instance

Here is what I was taught. Well-appearing young man. Hyphen. The patient is well appearing. However, I have had some QA say the latter should also be well-appearing. I guess go with company rules. I personally do not use the hyphen after a verb.

I'm so confused about this.  The patient is well-appearing.  Should there be a dash there even though it's after the verb?  I think there should be because you can't be appearing so it seems like you'd have to make a compound word there.  I think the BOS backs me up on this but that's a little confusing too.  I know it shouldn't be there if the patient is well known, well hydrated, etc...Anybody know for sure?

Under Review of Systems, it sounds like the doctors says "Derm no complaints, Hem no complaints." I've never heard this dictated in the Review of Systems before.

SKIN:  Lots of seborrheic keratoses.  He does have an elevated 9 mm papule on left side of the face, which has some vascularization and central core.  It almost looks like acanthokeratosis, but basal cell carcinoma cannot be fully ruled out.  No other suspicious lesions.  He does have some miliary rubra on the back.

I have 2 dermatology - I am usually not a derm girl, so any help is MUCH appreciated.


Assessment:  Nevus s/l: roo-let-ee s/l: tip-e-er midabdomen.  (Nevus ruletti tipiar midabdomen)?? AND:


s/l: Dermatified (or dromatified) broma right shin.  She uses this phrase in a previous sentence:


On his right shin he has superficial change that appears like a dermatified broma; however, it is not as nodular beneath the shin, but it is definitely excoriated or irritated on the surface. 


I just cannot find any info on these two - sorry this is so long.  I am covering for someone and would prefer not to send to the editor!!  TIA
