The patient has a shoulder SLAP tear with _____ glenoid cyst formation. 

All I can tell you is that the blank starts with an "S" and MAY sound like sphenoglenoid but not real sure.  Any ideas?  This is an ESL and he is awful!

I'm typing up the description of the operation of a suprapubic tube insertion and it sounds like they are saying:

A scalpel was placed in this area, and a Bard 12-French suprapubic catheter was passed into the bladder

My question is.......does it make sense to say a scalpel was placed?  The dictation isn't really clear and it could be a different word. 

Also...it was hard to find information on Bard catheters.  Has anyone ever transcribed this term before...maybe they are not really saying Bard.......

Hmm.  What could this be????  Anyone??

The patient has just undergone a radical nephrectomy and "oncu pain ball insertion".   Any ideas?  HELP!!

The ????? at the posterior tibial insertion certainly alters the pull of the posterior tibial tendon causing the functional pes planus deformity in his foot.  He is unable to adequately lift his medial longitudinal arch throughout midstance, leading to this pain at the posterior tibial insertion, weakness at the posterior tibial insertion, as well as plantar fasciitis. 

It is noted on X-ray.

Help..... Sounds like meniscus. Very clear.

The HUMI was then placed without complication. We changed our gloves and went up above. An incision was made in the infraumbilical portion with the scalpel. A Veress needle was placed and no complications with that. The *meniscus test was positive. Carbon dioxide gas was used to establish a pneumoperitoneum with approximately 50 mmHg with 2.1 L of gas.  OB/GYN

Patient found to be hypotensive upon entry to the operating room. "Mass trousers" were placed. CPR was resumed.

CT scan of the abdomen and pelvis showed a small s/l fat containing umbilical hernia.

the cervix was prepped with acetic acid and s/l a few or a-sea-a white changes were noted in multiple areas int the 11:00 region..etc.  thank you

5.  Parasystole S/L a-? relation

she does have some s/l hemi -actric' fracture blisters that are in that area.
