If you are transcribing, the correct way is:
V code: Parent-child conflict.
Unless you are a certified coder, it would be wise to avoid inserting the actual code into the transcription as the MD/psychologist did not dictate it. The codes can change from year to year and you could enter the wrong code depending on the date of the service and which coding resource you are using.

I've come across a dictation that is talking about NIDDM. It sounds like he's saying "he was initially of sulfanuria and switched insulin in Puerto Rico". The other part is "He has never been on metformin or glybizone." Any suggestions??

Actually we are in some cases responsible for the content. It is the opinion of a nurse friend of mine that we shouldn't be since we are not in the chain of responsibility. However, when a doctor dictates Synthroid 100 mg do you type what he says? What if the BP is 210/100 and he says hypotension. We don't type that if we don't want a poor audit and in some people's cases, poor audits equal pay cuts. So yes we are responsible for pertinent medical content. In my opinion if a doctor says something rude or ignorant, leave a blank and let him type it or rethink what he said.
