drug dosing
What should I do when doc dictates drug dose that is not in drug book.
ex. metformin 875 mg
When I look this up it only comes in 500, 850, or 1000. This is the first time, usually it's 500 or 1000. Should I just change it to 850 or leave it for QA?

What is the latest AAMT BOS standard for drug dosing such as "q.4h." I thought I heard that it was changing to "q.4 h. or q. 4h." Can someone clarify this for me? Thanks!

If a doctor dictates "Invanz 1 gram q. 24" would you put q.24h.or would you leave it alone. I know for sure she was scheduled for once a day. Is there an industry standard on this particular happening or is it the personal preference of each facility? Thanks!

It sounds like either one would work.
You'd only use hyphens if it modified, like in every-other-day dosing.

has anyone ever heard of a word parous or perous??? doc is giving list of meds and says aspirin 81 mg parous or perous daily, amlodipine 10 mg parous or perous daily. Definitely not saying p.o. and he keeps saying it, before the word daily everytime. This is a new one on me. Thanks

This PA is prescribing an antibiotic for an infant. Diflucan. She is clearly stating mm instead of ml. ? I have never heard of using mm. Am I just mssing something here.

Just wondering, has the rule changed in that gray must by abbreviated to Gy? Nothing in the BOS2. Nothing in CP. Account is verbatum. Doc says clearly gray, and since I have been typing him for 3 years, it has never been changed. QA had changed gray to Gy in an otherwise flawless report that I don't know why was sent to QA to begin with (this dictator isn't under 100% review and neither am I). Am I missing something, or is this QA on a power trip. I love QA and they do wonderful stuff, but, there are some who...well...may need to remove their heads from their bum.
