I'm working on a file about an HIV patient who also has a MAC infection.  Under the "IMPRESSION", the dictation sounds like.  Situation further complicated by disseminated MAC infection which led to s/l INCESNAL obstruction due to severe s/l MACROLIDED lymphadenitis.  Improving s/l TEKEXIA.

Any help anyone could give me with any of those terms would be appreciated.

Thanks

Listen to the voice file
Subject: Listen to the voice file

I keep all my voice files for at least three months. If I happen to get a QA tif back, I always go back and listen to the voice file and compare. Find out what the policy is on that account. I have found that some accounts expect corrections and others, right or wrong, expect verbatim transcription. I am blessed that the accounts that I edit and transcribe allow me to use good judgment.

Subject: Probably talking about report physicians file with Motor Vehicle Dept - sm

When patient's have seizure disorder or passing out episodes, etc. most states want physicians to fill out a "Confidential Morbidity Report" (or name similar to that) and send to the DMV letting them know the patient has been diagnosed with condition that can cause loss of control of the vehicle -- did doctor say he counseled patient on not driving until seizures under control for x-amount of months or something like that?

they put in tissue expanders before actually doing the tissue expansion in which patients return for fills to the expanders until they are at a proper volume, usually at which point the expanders get changed out for permanent implants (gel or saline).

Medical spell check and word expander
Subject: Medical spell check and word expander

Hi, Could anyone tell me, please, where I can download a GOOD, but CHEAP Medical spellchecker and word expander program?  I just got a new computer with Vista and my Shortcuts program or dictionary (Dorland's) will not work.  I have been off work for a while and need desperately to work, but I'm lacking money to buy those.  Info would be appreciated.  I've spent hours looking on the Net.  Thanks!!

I can only find "Wise" anywhere I have looked or searched. The doctor spells it as "Wyse" but I am not sure that is correct. Did you find it written somewhere? I want to be absolutely sure I am right before going against his spelling.

I am thoroughly confused on this! I have searched through everything, and find Micafungin. I can't figure out why, even though he pronounced it "mico", QA would change it to Micofungin. The dictator was discussing the meds he was using (ID). Any ideas?
