I figured this out because my mom was gravida 4, 2-2-1-2:  First pregnancy resulted in a miscarriage, second full term, third premature twins who did not survive, fourth full term.  So it could be that one of the pregnancies in your example was a multiple birth.  Of course, it's always possible for the doc to screw up the numbers, like when they try to number diagnoses...

Pressor support is the use of adrenaline-like medications to maintain the blood pressure and cardiac output above a certain level. Usually used in patients who might otherwise have too low blood pressure or go into shock.


Pressure support is the ventilator.

This is for a patient who has severe depression.  It sounds like the doctor is saying they will start her on Prestique 50 mg daily but I have never heard of an antidepressant that is even close to that.  Any ideas?

In current med list, the doctor states what sounds like "Celexa 200 mg daily" Since the doc already listed Celexa 20 mg daily earlier in the list, I would guess that she's not just misreading the 20 mg into 200... But my brain is so stuck on that it sounds like Celexa (even though I know it's not) that I can't hear anything else.

If it helps any, patient has GERD, PTSD, fibromyalgia, carpal tunnel syndrome, glucose intolerance, and hypertension.

Upon further research, I found that a lipoma could be found on routine CT or MRI without having any symptoms. So lipoma it is. Thanks very much. (On careful listening again, he's pronouncing it lapalma)

An EMG exam has been done in her lower extremities on 05/11/07, and this shows a wasting or "rapid oneal axonal neuropathy ".

Good morning...doc is dictating consultation and states

Cervical spine x-rays have been obtained, which revealed no evidence of cervical rib.  Documentation did reveal findings of diffuse degenerative disease in the cervical spine with pronounced osteophytic growth in the s/l angioportions of C4-C7. 


 


Is angioportions correct?  I cannot find good documentation.

EXTREMITIES: Strength is 5/5, normal motor strength in lower extremities including proximally and distally. Tone is normal. No cogwheeling. Deep tendon reflexes are equal. ("blanchers") are down going bilaterally. Coordination is slow but intact from finger to nose, bilaterally. She has no pronator drift; however, her left arm bobs quite a lot on out-stretched hands. Sensation is decreased on the right, especially on the new foot and hands, as well. Romberg is not even testable because she cannot stand straight with her eyes open much less closed.

HPI:  Workup included a noncontrast CAT scan of the brain which demonstrated generalized atrophy with S/L: EX-VAC-YOU-EL enlargement of all of the ventricles.
