syndrome and involves the wrist. It is a pressure test, but the doctor would hold the distal or proximal phalanx in the performance of that test while compressing the wrist and waiting for numbness to occur. I would like to hear the rest of your sentence before saying for sure what you are hearing is Phalen's.

Subject: Pack-a-day smoker versus pack-per-day smoker.
It would be either "He is a two-pack-a-day smoker" or "He is a two-pack-per-day smoker.
If doc dictates "a" use that; if "per" go with that. That's what I do anyway. It makes life simpler that way. :)

In my abbreviations book metacarpophalangeal and metatarsophalangeal are both listed under MPJ. Like you, when they dictate MPJ and it needs to be expanded, I have both codes mcpj and mtpj to avoid an error.

Subject: SR is sustained release and ER is
ER is extended release. It appears ER is the official way with glipizide, and yet SR would be just as clear to other MDs. So if you are supposed to correct, use ER. If you are supposed to transcribe verbatim, just give it to him, IMO.
