QA here: 1.8 thousand (1.800)is typed 1,800 or you can type 1.8K (with K the appropriate approved abbrev for thousand). Personally I use 1.8K.
If it was 18,000, it would be 18K.

Do not, however, insert the decimal point and zero if they are not dictated.

That's the way I was taught - never add a zero after the decimal unless it is dictated. (Title of post copied from BOS quote above.)
Doesn't change the fact that .012 needs a zero before the decimal point, does it?

We see numbers like 122/58, 125/59, 127/59, 130/70, and 120/60, with these blood pressures being consecutive "set" for a month-and-a-half with not a single elevated one among them. Does "set" make sense? TIA

I have had doctors, very brilliant ones while practicing their craft, speak horrible grammar, and I have been told by many, many supervisors and HIM personnel, even in college, that if a doctor uses the correct term but just the incorrect grammatical form, as long as you check with your supervisor, it is correct to make that one change, you are not changing the context one bit, nor changing the doctor's medical knowledge, just making it read much better, seem much more intelligent, for the doctor's wellbeing, as other doctors, personnel, lawyers, etc, get to read the report, and every doctor wants to be well-represented. I have a dear, very old Swedish pediatric gastroenterologist/professor who often will say "poops" when fishing for the words stools or defecates....my boss is more than happy to have me replace that appropriately, as is the doctor himself, ESL is tough!

Is there a PCP or specialty doc? If so, the letter would be to him, with other doc cc'd. If it doesn't matter, then I'd put the first one dictated as the main addressee and cc the other.
