You are right, there seems to be a discrepancy in administration route, if that is exactly how it was dictated. With a touchy person, I might try:
Please review possible discrepancy in route of fentanyl administration.
Something like that. Polite, yet not frankly pointing out a mistake, just saying a mistake might be a possibility, up for him to decide with a review. Hope that helps.

Not sure if Doc is dictating Ativan or something else....they are weaning the pt. off propofol and switch to *Ativan and fentanyl*.
Isn't Ativan used for anxiety? and isn't fentanyl a really potent narcotic? would they be using them together?
