well, I've been doing this work for 30 years and SOAP format was a big deal in the late 70s-80s too, but for URBAN hospitals and surgeons, they rarely use the SOAP format. Don't know where your *clinics* are (small town?) but in big big cities they have trended away from the SOAP format, for the most part. It's a rare SOAP I get over years. Also, the private surgeons I work for (5 of them) never use that format. I suppose hospitalists might but mine do not.

that is why they are there, but it is not just diabetics and not ALL of them have hypertension. Just because you are a QA, or think you are, does not make you 100 percent right on everything. You have made some very misleading statements on this site, just today alone. You really need to step off the arrogant attitude for a while and do some more research on A LOT of subjects before you start *generalizing* every certain disease by saying they ALL have high blood pressure and they are ALL in the hospital. You also need to learn to admit when you are wrong and become a little more humble in your dealings with others. you might actually learn something.
