I would likely say this is an anvil grasper, given the context and type of surgery. If you listen to it again with this word in mind, you may hear it.

I'm aware of an anvil test which is essentially hitting the calcaneus inferiorly (driving superiorly) of the supine patient to assess for a fracture (described as a femoral neck fracture). I guess at astretch you could extend this to fractures about the knee.

I say that it is spelled peon and yes there are peon graspers that are used laparoscopically during hernia repairs.
