Is cerebellar tonsil singular or pleural?  This could go cerebellar tonsils noted or cerebellar tonsil is noted as follows, and thanks for the help:

Inferior positioning or herniation of the cerebellar tonsils noted below the level of the foramen magnum into the upper cervical spinal canal posteriorly, consistent with Chiari I malformation.

Resident stumbling around dictating a T&A, making this much more difficult than it has to be!!!!!!

What is he trying to say?

My question is he is just saying 45 hole, not saying 4.5 hole so do I assume he means 4.5?  I get so confused by this.  This is the sentence leading up to my question. 

A 4.5-mm syndesmotic screw was now inserted using standard AO technique.  I placed the screw in lag mode.  A 4.5-mm drill bit used on the fibula.  A guide was placed in the 45 or 4.5?? hole.

The doc says a couple times that a well-padded tourniquet was used with what sounds like "webro" beneath was placed around the patient's left upper ankle but not inflated at this time.

Just cannot get the word...please help? Thanks

Dr says...A proximal, distal, medial and centrolateral s/l hemi-triced transection (Hoke technique) was performed.

Procedures are total ankle arthroplasty, tib-fib arthrodesis, percutaneous tendo-achilles lengthening, application unilateral external fixture, local bone graft, and fluoroscopy

The patient is having an examination after having closed manipulation and reduction of the fracture and percutaneous cannulated screw fixation of the fracture site.  This comminuted fracture of the body of the calcaneus remains in good positional alignment and with one "can sell us" screw and washer. 

Any clues on the screw name?

There was some question of a s/l 'dorsed chip of the vendicular' ??

Neurovascular appears intact with full range of motion of her ankle.  No evidence of tenderness except somewhat up at the  s/l pes an za rine.  Any ideas?

Hi, I'm writing a H&P for work and I need help!! The paitent is having ankle pain,

There is really no bony tenderness to the medial or lateral myleal(??) line. What "line" is this in the foot? Sounds like meleal/mylea line? Any help would be greatly appreicated!

Three views of the left foot demonstrate calcification in the peroneus longus and/or brevis, as well as two s/l suit jackets or suture jacks remaining in the posterior calcaneus

I could be way off, but that is what I am getting out of it.  Thanks

Doc dictated MCL, LCL, and Hoffa's fat pad information in an MRI of the left ankle.  I have been doing orthopaedics for a long time, and aren't all of these only in the knee, or have I learned something new in my old age?

This is an ESL doc.

She says.....Patient has some joint effusion.  Patient has s/l well-"corticated" multiple densities adjacent to tip of lateral malleolus and also over medial malleolus.  Seems to be well-"corticated".  Seems to be from old trauma.  Patient stated he sprained his ankle in past and he had x-ray done. 

Help!  Can't find anything even close!

She has 1+ (s/l) sacodema and 1+ ankle edema.

What is the 's' word?
