TMGE type pain?
Subject: TMGE type pain?

Dr. Says:  She states that she was diagnosed with severe TMJ and that her ear had looked fine, and she also saw a doctor for an ENT consult.  He agreed that he felt this was a jaw issue.

ASSESSMENT:  TMGE type pain, also some spasm in her left neck.  It seems like the sternocleidomastoid muscle area. So, it does seem like TMJ syndrome.  (What is TMGE?)

he pain of the anesthesia injection itself can be minimized by injecting the agent slowly and steadily.[4,5] The addition of 8.4 percent sodium bicarbonate to the lidocaine (ratio: 1:10) appears to decrease the burning sensation during infusion, presumably by raising the pH.[4] The correct angle for a subdermal injection is 30 degrees, which will smoothly raise the lesion from the skin[2] (Figure 1). If the injection is too deep, the lidocaine will diffuse into the fat, and a wheel of anesthesia will not form.
