You may want to bold the "Addendum:"  Then type as usual.  This often happens at the end of the report and should be placed there.  At least this is how I have always done it.

Subject: You might not like this one, but I would go by what QA said on the previous report. This way you have verification in writing of why you chose to keep it this way in case you are questioned on your choice of interpretation. I know we are looking for common sense but we are transcriptions and as we know that is not even close to always being the case in MD dictations.

I took this from a previous post about eponyms... sm
Subject: I took this from a previous post about eponyms... sm

According to BOS

eponyms

"While the use of the possessive form with eponyms remains acceptible, AAMT's preference is generally to drop the possessive form.

Apgar score, Babinski sign, Down syndrome, Gram stain, Hodgkin lymphoma.

Sometimes an awkward construction calls for use of the possessive form. This patient suffers from Hodgkin's."

on this same question.  Is this an ENT account?  The best thing would be see if you can get verification from the account, might be some name they have for their own conconction of Maalox, Benadryl and Xylocaine.  I've seen plenty of things misspelled in reports, even standards handed out by an account itself...

Perhaps that poster was in the middle of a report.......so one wants the answer ASAP...but for years I mistyped Mohs....thought it was MOHS.....you know, like a flap in surgery.....and alot of them are all caps.....then my friend had to have Mohs surgery and I learned.  The great thing about MT'g is that we are continually learning new things.

type verbatim. The doc is saying pussy and that is what she should type.

If you look in the dictionary, there is a definition for pussy which means "full of or resembling pus".

You can't say "no way is "pussy" correct. You are telling her to alter the sentence and she said it is a verbatim account. It is a word.

If it is not verbatim, then alter away but you better be careful if you do this a lot.
