I am not absolutely sure about the M (could be N) or O (could be L), but I know it is an accronym as he says:


"discuss end-of-life care and capital M, capital O, capital F, capital T paperwork."


I cannot figure out what kind of form this is or what the accronym stands for. He says it so quick I am not even sure I have the right letters!
