I am typing a letter to a claims manager, and doc states

"He does have a deposition coming to s/l furlation in the near future.

How do I address the letter/envelope when MD dictates that the letter is to go to:  (Drs Max, Jones, Smith, and Brown.)  All of these MDS work in the same office together. 

I need help formating this the correct way.  Thanks.

Hello, I am writing a letter and doc wants me to list this info as a chart in the letter? In 2004 225 pounds, 2005 242 pounds, 2006 267 pounds, 2007 258 pounds, 2008 255 pounds.  How what I format that?

I am thinking....

In: 

2004  225 pounds

2005  242 pounds

etc?  However, it would not be double spaced for some reason it keeps doing it on here?

If it is business correspondence, I would use the person's "official" first name. If it is a personal letter, I would use the nickname.

If you're not sure, I would advise leaving it blank.

My MD dictated a letter of recommendation and would like me to make him sound better.  Gosh!!  I am so brain dead when it comes to these things... been doing verbatim transcription for far toooo long. 

Would anyone out there like to give this a whirl?

Mr. X spent a week with me in my internal medicine office this past summer.  He is currently a student at XX University and has decided on a career in a medicine.  Mr. X, in the process of starting his medical career, has spent time with multiple physicians to show that he is serious about a career in medicine.  Mr X has excellent personal skills, as you will see when you interview him.  He asked intelligent questions and showed a genuine interest while he was here with me in the office.  His thought process seemed both logical and rational during our discussions.

Mr. X  has all the makings of a good physician, intelligence, hardworking, motivated, and personal.  I recommend highly that he be accepted in your training to become an xxx  physician.

The studies demonstrated carotid artery disease but, also, she has a platelet count of 1004 k per cmm and persistent reduction in her hemoglobin and hematocrit at 9.4 and 31.5.

Does anyone know how to format this?  I can't find the K at all in relation to these values, but doc is  clearly saying,  1004 K per cmm.  thank you.

This was 250 pounds, (s/l gross volume to) body mass index of about 40.

I never usually have to type letters so I am unsure of the format.  Are we supposed to type the "Sincerely," and are we supposed to indent it?

but it DOES depend on the client profile/preference like the other poster said.....I get this format all the time in REAL consult reports (not letters) and I just type *Dear Dr. Smith* - I have just seen your patient....blah blah (can usually go under reason for consult or HPI) and then do my headings as dictated.

She had two or three episodes of this two days ago and then probably six to eight episodes yesterday, and then one or two episodes this morning.

Wouldn't they all be spelled out or would "episodes be considered a technical term"?
