How do I address the letter/envelope when MD dictates that the letter is to go to:  (Drs Max, Jones, Smith, and Brown.)  All of these MDS work in the same office together.

Regarding letter h (hour) per BOS...please sm

Letter h. (with a period) is the abbreviation for the Latin word hora, meaning hour.

every 4 hours or q.4 hours or q.4 h or q.4 h Copyright (c) 2002 American Association for Medical Transcription.  So does this mean we can do the q.4 h without the period at the end of h, because I am constantly having to remove capital letters from words following the h with a period.  Thanks.

The doc wrote a letter to a physical therapist on behalf of a patient. At the end, he says to copy to the patient and include a copy of her MRI scan results. How do I transcribe this so that the doctor's office knows to enclose the MRI results to the patient, not the physical therapist who the letter is to?

If it is business correspondence, I would use the person's "official" first name. If it is a personal letter, I would use the nickname.


If you're not sure, I would advise leaving it blank.

Mr. X spent a week with me in my internal medicine office this past summer.  He is currently a student at XX University and has decided on a career in a medicine.  Mr. X, in the process of starting his medical career, has spent time with multiple physicians to show that he is serious about a career in medicine.  Mr X has excellent personal skills, as you will see when you interview him.  He asked intelligent questions and showed a genuine interest while he was here with me in the office.  His thought process seemed both logical and rational during our discussions.


 


Mr. X  has all the makings of a good physician, intelligence, hardworking, motivated, and personal.  I recommend highly that he be accepted in your training to become an xxx  physician.

The studies demonstrated carotid artery disease but, also, she has a platelet count of 1004 k per cmm and persistent reduction in her hemoglobin and hematocrit at 9.4 and 31.5.


Does anyone know how to format this?  I can't find the K at all in relation to these values, but doc is  clearly saying,  1004 K per cmm.  thank you.

K is abbreviation for thousands; also used as abbreviation for potassium. platelets are expressed in thousands.

I think the 1004 k/cmm should be expressed this way.

but it DOES depend on the client profile/preference like the other poster said.....I get this format all the time in REAL consult reports (not letters) and I just type *Dear Dr. Smith* - I have just seen your patient....blah blah (can usually go under reason for consult or HPI) and then do my headings as dictated.

It would just be "ug" - u stands for the symbol for the Greek letter mu, but (sm)

Thanks for replying so fast.  Actually, my question was on the J point elevation s/l "1 and ?"  Kind of sounded like 1 and the letter "o", but doesn't make sense.  Didn't know if maybe he was speaking of a certain lead, but doesn't sound close to anything.  Thanks so much for your time!!!!  :-)
