So I guess it's not much of a stretch when you're talking about a fusion to say L4-sacrum.


Docs invent their own words - they don't care about what's "correct". A lot of times it just depends on whether the account wants everything verbatim or if they expect you to clean things up and put them in proper format.

I capitalize them when they are in the diagnosis, but if they are generally talking about it in the body of the report, like,


"I explained to Mom that obsessive-compulsive disorder would be difficult to assess."


Is it Obsessive-Compulsive Disorder in that case?
