I've also received the same instructions about cath, subcu, exam, etc., but can't remember the source. It might have been a past company's specifications. I know that these words MUST BE EXPANDED (per BOS) in diagnoses, operation titles, etc. (same rules as abbreviations). This might be a new HIPPA rule also which came out after the second BOS was printed. I'll keep thinking. But he's not to be flamed. I've heard the same rules and don't mind using them because it looks more professional and gives me a higher character count.

ASSESSMENT: We discussed options for Jeff. Also, another sentence begins with "4-0 Vicryl was also used..," what do I in this situation? If they are strict on verbatim, just go with number? 4-0?

I always thought q. daily was wrong, but have received back corrections from QA with q. daily after I had changed it to daily. This is not a verbatim account. Am I wrong?
