Doc means below the 1st metatarsal bone, so you have to put the "sub" at the very beginning of the whole thing (not 1st submetatarsal).
Use a hyphen, sub-1st, because sub- is a prefix not a word.

The conservative means of therapy in this case would be orthotic devices with a deep s/l-rale- for the lesser metatarsal heads and support of the metatarsal necks.

It's a missing foot. I'm thinking it's another word for phantom pain but I haven't been able to find anything. I've listened many times and it still sounds like "tre-eye-n". Or maybe plural. Later on he says it's not vascular but may be neurologic or musculoskelatal in nature.
