EXAM:  No murmur or gallop noted.  CT scan of the chest done 03/17/2006 revealed a 2.5 cm soft tissue density left of the distal esophagus.  Also a retained catheter fragment in the left lower lobe and (s/l bilinear) fibrosis of the left lower lung.

Of course, the first dictator I get today has to be *thee* worst one in the clinic... at least with my experience. I'm training and I do not want to have another blank against me.

The patient has abnormal liver enzymes, hiatal hernia, mild reactive *something* with the GE junction with focal erosions, and mild antral gastritis with a few erosions and something else I couldn't hear.

WORD I NEED HELP WITH: Sounds like "dumb-it-fans dumb fibrosis." LOL Anyone have ANY idea what he is talking about? Thanks much!

I think if you would substitute "bilateral"  (mentally)  and say  "both" you might have a better idea how to say it.   Like would it sound better iff you said "both hand"  or "both hands."   To me bilateral hands sounds better.  But again, I don't know if there is actually a rule.  I looked for an answer in "style Guide for Medical Transcription" (Tessier and Pitman) and couldn't find an answer.   


I think if you wrote what the doctor said ....you can't go wrong there either.  Good luck!.

if you have a credible reference, spell it whichever way the reference spells it.
you should never be counted off for it, as long as the reference is credible. if you are, it is a valid challenge to assert.
