The eyelash was still within the eyelid itself. I used magnifying glasses, as well as splinter forceps and was able to localized the eyelash and remove it from the lower lid. Sometimes they say something like this: "A registration error of 3.1 mm was obtained. Surface matching registration was then performed on all levels." So, if they're not giving you a specific number, like 3.1 mm, then you would spell out "The millimeters of error were registered..."

While the gastrocnemius and soleus are 2 separate muscles, they come together in a common insertion to form the Achilles tendon, hence the frequent references to the "gastrocsoleus complex." The term "gastrocsoleus" would not be seen in Grey's because the information deals with the anatomy of each muscle individually, but that does not mean the two cannot be referred to and transcribed in the above manner.
Stedman's shows it as "gastrocnemius-soleus complex" while HPI Orthopedic Words and Phrases makes a reference to "gastroc/soleus." I have typed it as one word for many years and have never seen any QA objection to it. The bottom line will be, if your particular company or account has a preference, then go with that. In the absence of such a stated preference, however, it is acceptable to transcribe "gastrocsoleus" when dictated. Some will view it perhaps as slang or whatever while others will see it as merely constructing a medical term as done within the acceptable parameters of medical terminology for the purpose of referencing anatomical structures that share a point of commonality.
Cardiac cath questions, deleted posts? sm
Subject: Cardiac cath questions, deleted posts? sm
Some posts just got deleted as we were trying to help someone. My answer to her format question was: Some of my docs do dictate each artery's findings separately and I number them along the left under each other. Just use your common sense so you end up with a clear report. Good luck. I remember when I first started doing caths and it wasn't easy.
