grades of motor response is in Arabic, not Roman, on scale of 1-5, with plus and minus often include, often expressed as for example -4/5 or 4+/5 or 4-/5, depending on style of physician or account. You type what you hear, i.e., Grade -4. I first Grade and Stage. Hope I've been helpful to you!
