OBJECTIVE


VITAL SIGNS: Weight 142 pounds, blood pressure 120/92, temperature 98.2.


GENERAL EXAM: In general, the patient is a well-developed, well-nourished, pleasant 47-year-old Caucasian female in no acute distress.


SKIN: Skin is warm and dry.


HEENT: Eyes: Sclera are white. Conjunctiva: pink. Ears: Examination of the left ear shows the tympanic membrane to be retracted and dull gray.


 


I cannot tell what type of transcription he is doing so that I can format it correctly. It starts by saying that the patient is blank years old and is seen on blank day for service. Then he states CHIEF COMPLAINT, SUBJECTIVE, and then the OBJECTIVE.


 


Would you list the above like I have or just type is out......


 


This is so diferent that the ones we were made to do at school! VERY frustrating!!!!
