That is what's happening right now with HMOs. How many times have you typed a letter for a doc to an insurance company appealing for the 3rd or 4th time to allow treatment for a patient? I know I've done tons of them. They are not appealing for plastic surgery or elective surgeries, they are appealing for cancer treatments, etc.
