so you get a dial tone, that means it is working. Next you dial the number of the dictation system, and it says something like "welcome to bla-bla hospital. Please enter your user ID followed by the pound sign." Then you enter your ID and it starts giving you work in your queue or asks for job type or whatever.

To clarify, the dictation is likely recorded at a hospital...
and all the C-phone or Lanier does is access that system. Any hospital that uses a Dictaphone system can be accessed by a c-phone. Seems to me there are some electronic units on the market that can be either Lanier-compatible or C-phone compatible though you would have to ask around about that. Yes, their only purpose is to access dictation that is stored somewhere else...though you could use the C-phone as a regular phone, if you wanted :>) So there are two types of data format nowadays, .wav files (or .dss files) which the file can be transferred via internet and dictaphone/Lanier format which the file has to go through a phone line. And perhaps a few microcasettes are still kicking around. Any other old-timers have anything to add?
Mostly inpatient hospital dictation - H&P, DS, Consults, Ops, some include ER
and progress notes. Acute care is usually inpatient dictation from hospitals.

Gross line means anything on a line is a line, even if just one word
it is counted as a line. Not sure what exactly you are asking, but if you are comparing gross line to the usual 65 characters plus spaces gross line is better. If you make 7.5 cpl per gross line that equals approximately 9.5 cpl based on 65 characters.

Multispecialty Clinic
Your experience sounds more like multispecialty clinic work than acute care and you would probably be much more comfortable with clinic work at this time. Unfamiliarity with acute care will definitely slow you down, which will ultimately cut into your paycheck. You might not have a problem doing work for a small hospital with limited procedures and limited specialties, but any larger hospital or teaching hospital is probably well beyond your skill level right now.
So it's better to start out in the clinic
area rather than the hospital setting? How do you find clinics who are hiring? I look in the paper and I don't see any.

If you do clinic work, I'm sure you
could find ophthalmology, but if you need a dependable income I'd suggest you not be that rigid.
One characteristic of most good MTs is that they like learning new things. There used to be a lot of money in repetitive reports, but less so now with VR and EMR technology, and foreign countries doing the work for lower pay, there isn't as much of that anymore.

Spheris hires Career Step grads but only at 0.055, (at least you'll be home) and probably won't make more than 150 lines an hour, so you'll start out there making about $8.25/hour, they're clinic platform is easy, they have flexible hours, supervisors and QA people are good, they pay on time.

I'm sure they could direct you to the person who interviewed you or trained you. I have read about things like this happening here on these boards even to the experienced MT's. I can't see how a phone call would hurt anything. Just shows you want to work, IMO. Good luck.

clinic versus acute
acute care consists of patient care in the hospital from the time they are admitted until they are discharged (emergency room, history/physical, consultations, progress notes, lab/imaging data, transfers, and discharges are the most common). clinic notes are office visits (sometimes include consultations and maybe lab/imaging data). these are usually shorter dictations than the acute care.
Is Multispecialty Clinic paid the same?
Or is it less? I've done H&P, Consultation, and Discharge Summaries. Just no Op notes.

my multispeciality clinic has 28 specialties
GI, GU, cardio, ortho, derm, ENT, surgery, endocrine, plastics, audio, ophtho, podiatry, physical therapy, OB-GYN, internal medicine, pulmonary, sleep center, oncology, infectious disease, pediatrics, urology, nephrology, allergy, rheumatology. Those are all the different specialties you can learn in clinic.

that's a very personal call --
no one can make it for you. There are good jobs to be had. But it is true the industry is changing as well. Did you ever have the varied transcription knowledge? Not sure if i would want to go back to school either. But then, there aren't many decent jobs you can do at home either. Maybe you can find a company that will work with your current knowledge while cross-training in other specialities, without having to go back to school. As far as what you see on the posts, that can be very helpful, but don't let it discourage you. I am sure there are many many MTs that are happy and never even look at these boards. Just decide what you want and go for it. Just don't think there's only one way to go about it. With all the changes, i still think there is a future for us.

leave one blank every other report or so. Usually it's due to some random phrase they're not used to hearing. It's rarely an actual medical term.
I've got a week or so before I call...
...them back and accept, but if they're hiring more, I'll post it. My advice is to just apply like crazy! I applied even to places that said they wanted full-time MTs. Why not? You never know. Maybe they'll have that little "extra" work and will keep you in mind, or you might just hit them at the right time and place. Good luck to you!

I'm in this business many years, almost 3 decades, and I STILL have a couple of people, one especially, who I call when I have an MT question or if I want them to listen over the phone....it's networking and it's great and if you find a REAL good mt or cmt, well, then you've got GREAT help. Sometimes 4 ears are better than 2. I even have my daughter listen if she is here (albeit she is not in the medical field except 5 years so far *S*)

staffing schools is what I call them
I don't want to mention who I work for because it is a small world among the larger companies and schools and I don't want any hard feelings or job grief. I'm too old now to start over!!
Medical Transcription Staffing schools are not the retail type of schools we normally think of; the ones who advertise on Google, Yahoo and MSN. These schools specialize in training groups of students for an MT company all at one time. Or they provide small groups on a regular basis.
These types of schools also exist in other fields. They work on what I would call "wholesale margins" because they don't incur as much overhead and they deal in larger numbers.
Rather than promote who we use I'll find one for this board as an example. I can Google or Yahoo the web and should be able to find one. I'll do it this weekend, if I can remember.

You didn't mention the name of the company. It could be they have a new account and it's not off the ground yet. It was a month after I was hired before I actually started working. That was two years ago and I'm seldom out of work. I'm still on that same account.
This is a wakeup call for those with inadequate skills
As technology allegedly improves and new medical procedures and techniques are created, we all have to keep our skills up. Those who have not are having a tough time keeping job these days. They may want to blame it on the changes, but it isn't the changes, it's their lack of skills. We finally have come to the place in this industry that people can't just listen and type what the doctor says. Anyone who got into this business with less-than-excellent skills is now getting a wakeup call. I think that's what this poster is telling us. She has had a wakeup call and she has seen other unqualified MTs getting a wakeup call. Those who are prepared and continue to build on their skills have a present and a future in this industry.

What if the MD is dictating, you are a 1/4 of the way through, he is interrupted by a colleague, then comes back starting with the beginning again (prepped & draped, etc)then seems to go in a totally different direction and subject then where he left off? Do you continue to type what he is saying, even though it makes no sense to the first part of the report? Do you make a note of it for the editor? Any thoughts would be appreciated. I'm stumped on a sentence in a discharge summary I am working on. As part of the Laboratory Data section, the doctor says:
Chest x-ray showed no TECUTE PROCESSED. Well, at least it sounds like tecute processed but that obviously doesn't make sense! Any ideas?

Seriously, speeding it up just a little sometimes smooths it out. I got this tip from an MT with over 20 years experience. I wouldn't do it the first time through, but when you're listening again, try it.
Dictation volume
Hi all, I was just wondering if anyone else was having problems with the level of dictation available to transcribe. I work for Eagle's Landing and I'm finding that the dictation is very sporadic. When I first started it wasn't that way but ever since Hurricane Katrina the dictation has been low. Right after Hurricane Katrina there would be days with no dictation because the docs were gone to New Orleans to help out. Now the doctors are back but there is still the same problem. I am actually going to be set up on a second account so that should help me but I'm not sure when they are going to do that for me. I've heard that we are getting several new clients soon so maybe that will help. What I've started doing basically is actually keeping up with the amount of work per hour (low, medium, high volume, comp issues, etc)on an excel spreadsheet for each day of the week, I'm going to continue doing that for each week of the month, so eventually I will have several months I can look over and see if this is a chronic problem or an acute problem. I'm really hoping that this is just a temporary slow period of time. I think my frustration has also been compounded by the fact that I've had computer problems myself, the system at Eagle's Landing goes down sometimes for maintanence, and just being a new MT.
I really do plan to stick it out but I don't believe that the service would be honest with me if this is a problem that is going to be ongoing so I feel I need to keep my own records. Right now I'm living at home with my mom but eventually, in a year or so, I want to be able to move out and support myself so I need to see if this job has some stability. I understand there will be less stability than other jobs because that is how this industry is but I need enough stability to know that I can support myself atleast. As I said before in one of my earlier posts, I only started 3 months ago,(back in July) so I definitely know that the early frustrations just take time and experience to work through so I do plan on sticking it out. I guess my concern with lack of dictation is that it is something I cannot control, my speed is something I can control, I can work really hard and get better at that, but this is something I cannot. The problems with the sporadic dictation I would say have been going on since the middle of August, it does not seem like very long but it seems like forever when I'm trying to build up my speed and when I'm new at this. Anyway, I would appreciate some feedback on this from new MTs and more experienced MTs. Oh, also another thing I've noticed just from reading this message board and others is that a lot of MTs keep up with their line counts on a spread sheet. What I tend to do is watch my production based on the number of minutes I type. Should I be looking more at line count than minutes and also keeping track of that more closely? I have noticed that I do not follow my life count as diligently as others. And finally I want to thank everyone for the input I've already gotten from this board, it has been very helpful to me, and I have definitely taken the advice to heart and used it.
You don't record the dictation (see msg)
with the c-phone you dial in, and plug a foot pedal and headset into the phone itself and transcribe that way rather than hearing through your computer speakers. (I assume voicewriter is similar)
Practice Dictation
I'm looking for free practice dictation in oncology/hematology. I'm coming up with nothing on Google. Can anyone point me in the right direction? I've got a job lined up, but I need more practice before I start. TIA for help!
Practice Dictation
I am starting the practice dictation portion of my training. The first examples were someone from the school that I chose doing the dictation and then I moved on to actual doctor reports.
My question: How common is it for doctors to talk like they have a mouthful of marbles? The first one was awful and there was no way I could decipher what he was saying. I had my husband listen to it and he looked at me like WTH?
The practice is on CD ... does that make any difference? Are the files that are received better in clarity?
This is really disheartening. I feel lost and am beginning to wonder if I can even do this.
I loaded the first 20 practice files and about half are understandable. The rest is a just a garbled mess.
Any input is sincerely appreciated.
