most prices are per 65 char. Such as you type a report and the total character count is divided by 65 to get a "line count."  May or may not include spaces. Some accounts are better than others. For instance, if you can get a job where you do the same doctors over and over again, your line count per hour (therefore your wages) will increase substantially over an account where you are working with many different doctors and many different report types. The amount of ESL doctors you are working on also will affect your line count, especially as a beginning. I hear most newbies are paid somewhere around 6 cpl, although I also hear many experienced MT's are hovering around 6-8 cpl. However, as said, on an account with American dictators, much repetition, you could be paid 6 cpl and make more than an account paid with no spaces, different docs all over the place and many ESL at 8 cpl. through my list of hundreds of online companies that Career Step is the most endorsed school. The other two are actually hardly mentioned anywhere, except here and on their own school boards.
This varies with companies, but
most of them have a word test to test terminology and spelling and then some dictation as well. Some are timed and some are not.
What you need varies per company, but
you need a way to download the work, a foot pedal, and software to play the files.   The work is received via internet, usually via an FTP or VNP.  The foot pedal you need depends on the software.  Some companies have propietary software and you could need a serial port pedal or a game port pedal, though most you need  a USB pedal.  Sometimes the company provides pedal/software, other times you can use Express Scribe (which is free) and you need to provide a pedal. 
Time frame varies sm
I am finishing up session 5 this month. I started at the end of August. I complete about a session a month, which would put me at finishing in about 18 months total (there are 18 sessions to the course). I have heard of some finishing in as little as 10-12 months, but I think around 18 is the norm.
The time you will be able to spend studying would affect how long it takes--I have 2 jobs and work about 50-60 hours per week outside of my M-Tec studies.
Don't hesitate to call the school office with any questions--they are closed until after New Years, but are very helpful with any questions you may have. Good luck!
It varies greatly. Some companies pay per line. I've seen as low as
2.5 cpl and as high as 7 cpl.   Some companies pay by the hour, most in the $10 to $15 range.    You'll find exceptions in each case, but this is a ballpark amount.
