That's always a sticky problem

You're going to need to develop a thick skin and a pat answer to that sort of question from unqualified people. Since you seem to feel some obligation to reciprocate for information she provided about the tax preparer's job, maybe you can build on what you've already told her, but put a positive twist on it. Tell her you'll be more than happy to put in a word for her when she's finished the medical terminology course. Tell her there is nothing you can do for her until she can pass the terminology test. Explain that it is a requirement and has nothing to do with who knows whom. For others who ask you to get them a job, it's best not to offer any reasons why you can't--that just gives them something to argue with you about. Just tell them, "I'm so sorry, I'm afraid I can't." If they ask why, just tell them "I'm so sorry, I'm afraid it would be impossible." Do not try to explain about your background because they'll have some lame rejoinder like "But I watch all the medical shows on TV." You can explain the kind of training they would need, but it won't do any good. They'll just want you to get them in without any training or preparation. Just smile sadly and tell them it's just not something you're able to do. Then change the subject. Good luck in your new job.
