I suggest clearly delineating with your doctor(s)/customer(s) the rules of your own influence in modification. It sounds like they want it verbatim, so give it verbatim. If you change what the doctor is saying just to appear prettier (like improving grammar), then that is a frivolous and possibly dangerous action. If you think there is an ambiguity/misinformation inherent in the verbatim transcription, I would suggest changing it and noting it, or typing it as-is and noting it. The important part is to flag any misgivings you might have about it, to point it out to your doctor(s)/customer(s). Do not change it unless you are 100% sure, if then.

I did not go to school for transcription. But, I have spent a lot of time and research into communications/information theory, and I typed neurological transcription for about 1.5 years. I understand how tempting it is to change things "for the better" grammatically-speaking, but my lightning-quick decisions/interpretations are not as good as a doctor's interpretations would be. It concerns me that apparently people are taught that doing this is acceptable and even encouraged. If your doctor(s)/customer(s) says that it's okay, then I would suggest to have that in writing if at all possible, or at the very least have a clear discussion along these lines. I believe the health of every patient is worth the effort, but the ultimate decision is up to each and every MT.
